Includes a list of required questions and a short summary of the school year. Detailed Evaluations** - More descriptive evaluation, which includes a detailed summary of the courses completed. You can also download a copy from the forms page of this website.) * Travel Evaluations are available on a very limited basis. For locations more than 15 miles from my Harrisburg home, a minimum of three families per trip is required. I will also do group evaluations at a single location. Contact me early to schedule travel evaluations. *Online Evaluations option is also now available. Payment is expected prior to the meeting. For the online option, the samples and detailed Evaluation Worksheet will need to be uploaded to a google doc link which will be sent to you. Samples will need to be uploaded 48 hours prior to the scheduled appointment. A Zoom meeting link will be sent to conduct the interview part of the evaluation.
